Health Wellness and Nutrition

MPAC understands that community development is far more than building beautiful homes; it is about restoring and preserving the health of those within the community. Staying healthy is not just about medical treatments and procedures—it is also about prevention and a lifelong commitment to wellness. Conceptually, the Health Wellness and Nutrition Center will empower individuals to achieve healthier lives and healthier communities through primary care and sustainable life-long changes in diet, physical activity, and personal health management.  The Image of Health Wellness and Nutrition Center will empower patients to make informed health choices and to take responsibility for a goal of lifelong wellness through a healthy and nutritional lifestyle. Educational instruction will be offered to address the social determinants of health (SDOH) and integrate them into patient care.

A second, and equally important differentiating feature in the Center’s model from other health centers, is the incorporation of Health Literacy as an essential life skill.  Health literacy teaches patients the ability to access, comprehend, evaluate, and communicate information---both in-office visits and through technology---to promote, maintain and improve health in a variety of settings across the life cycle.
